Transcript Request

Graduates who need a copy of their transcript should send a WRITTEN request to the Guidance Office.

Please include the following information in your letter:

• Your full name and date you graduated
• Your social security number
• The address where the transcript should be mailed
• Request either an official or unofficial transcript
• Include an e-mail address, street address, or phone number where you may be reached in the event we have questions.
• Please enclose a check for $5 for each copy of the transcript, with the check made payable to Sherwood Christian Academy.
